New Patients
Osteopathic Vision is located at 515 East 750 North, Lebanon, IN 46052. We are in a green metal sided building on the south side of the road across from the Cool Lake golf course.
Our office hours are Tuesday – Thursday from 9-5. Evening hours or other days can be arranged. Appointments are necessary.
Please read the Treatment Tips to learn more about what to wear and expect from your appointment. If this is your first visit, please arrive at least 15 minutes early to fill out paperwork.
We are a fee for service clinic. We will provide you with the paperwork to file your own insurance claim. We accept Visa, MasterCard, check or cash.
I do my best to honor you and your busy schedule by running my practice on time. Please honor me by changing or canceling your appointment at least 48 hours in advance. The full fee will be charged for missed appointments.
There is ample parking in the lot. If you require assistance in getting into the office please call ahead.
